The method of payment by TODI to the undersigned ("You" or "Your") for services duly performed by You for

TODI pursuant to anyand all agreements between You and TODI ("Services") is through electroni funds transfer

via a payment system called ACH, also known as Automated Clearing House (collectively referred to as "EFT")

Please complete, sign and submit this EFT authorization form to TODI at info@trucksondemandinc.com along

with a copy of voided check from the bank account You list below. Upon signing this application, You authorize

TODI to electronically credit Your bank account for amounts due to You for Sevices (and debit your account only

for any payments made in error by TOD) You will not be charged any fees for receiveing payments for Services

via EFT. You will receive detailed payment remittances via email from TODI for the amounts credited to

Your bank account. Please note, US Currency must be credited to a bank located in the United States.
